　　We have an exciting opportunity to join our team as a Infection Preventionist - Ambulatory Clinics.In this role, the successful candidate Under the guidelines of the Director of Infection Prevention and theHospital Epidemiologist plans and implements daily operation of the Infection Prevention Program. Responsible for the surveillance, analysis, interpretation and reporting ofHospital acquired infections (HAI); educating employees about infection prevention; and the development ofHealth system policies and procedures to insure rigorous infection control standards that meet JCAHO, OSHA, Centers for Medicaid/Medicare Services, Centers for Disease Control and Prevention (CDC) and other nationally organized agencies recommendations and requirements. 　　Job Responsibilities:

　　Evaluates patient care practices for effectiveness, application to Infection Preventionist (IP) and provides feedback to the appropriate departments. Identifies and solves potential problems related to the implementation of IP practices and communicates to Director. MarketPromote IP program within and outside WUH. Acts as an IP liaison with public health authorities and represents IP on assigned committees. Assists in obtaining and maintaining accreditationlicensure and enhancing compliance with regulations and standards.Performs other duties as assignedParticipates in maintaining records and logs of reported diseases. Conducts community. Participates in Infection Prevention Educational activities of WUH. .Assess the educational needs of HCW and patients pertaining to Infection Prevention. Develops goals, objectives and lesson plans for educational offerings based on principles of adult learning. Prepare and presents educational workshops, lectures, discussion, or one to one instructions on variety of Infection Prevention topics depending on the educational needs of the HCW, patients and community education via health fairs with focus on identified problems of the specific community when assigned.Assists with analysis and trending of exposure incidents and Employee Health Services develop Infection Prevention strategies, to address the risk of Infection transmission between patients and health care providers. Coordinates contact investigations when healthcare worker (HCW) is exposed to communicable diseases (e.g.: chicken pox, tuberculosis).Under the guidance of DIP, conducts ongoing environmental rounds to identify potential infection risks of patients, staff and visitors and provides feedback and recommendations to the patient care units and support services; collects data on Infection Prevention related performance improvement activities; identifies Infection Prevention activities for P.I. projects (e.g.: hand-washing, transmission precaution practices, flash sterilization) and strategies for specific patient care settings and non-patient care departments (e.g.: nursing units, ICU, respiratory care therapy, OR, environmental services, food service department).Reports cases of communicable diseases to appropriate Health authorities. Identifies reportable diseases from laboratory report review, routine surveillance rounds and from different departments and patient care unit telephone reports. Reports communicable diseases to the Nassau DOH and NYSDOH and other agencies as required by law. Identifies and reports cases of specific communicable diseases as alerted by the Nassau DOH NYSDOH and participates in prevention and control of community outbreaks.Identifies Infection Prevention strategies to reduce infection risks associated with patient care activities, a therapeutic and diagnostic procedures and devices (e.g.: hand-washing, cleaning, disinfection, and sterilization) and shares to DIP. Identifies strategies for recall of potentially contaminated equipment and supplies (e.g.: manufactures recall, sterilization failure) and strategies for specific patient care settings and non-patient care departments (e.g.: nursing units, ICU, respiratory care therapy, OR, environmental services, food service department). Assures that identified patients are placed on transmission-based precaution are and appropriate isolation measures are instituted based on revised guidelines from agenciesIdentifies reportable diseases from laboratory report review. Reports cases of communicable diseases to appropriate Health authorities (i.e Nassau DOH and NYSDOH) and other agencies as required by law and participates in prevention and control of community outbreaks. Conducts community education via health fairs with focus on identified problems of the specific community when assigned. Identifies the Infection Prevention Implications of products and recommends products for safe patient care practices. Provides input to product selection.Conducts outbreak, Surveillance and Epidemiological Investigations and follows data collection methods. Compiles surveillance of data, calculates the incidence rates of HAI and device related infection rates and the prevalence of specific infections (e.g.: STD, VRE, MRSA, HEB, HEC); Reports findings of epidemiologic significance to appropriate groups, units and IC Committee. Identifies and classifies infections at various body sites (e.g. (NHSN definitions). Identifies (HAIs) and reportable diseases. Reviews laboratory reports daily to identify potential Healthcare Associated Infections (HAI), reportable disease, clusters of infection and isolates or recalls patients when indicated.Minimum Qualifications:

　　To qualify you must have a Bachelor's Degree In Nursing, PublicHealth or Allied Field. BS in Nursing with current licensure by New York State as a Registered Nurse (if hired after 11/17/2023). Minimum of two years experience in Nursing is required. Knowledge and ability to collect, interpret analyze and report on surveillance and infection data. Active Certification endorsed by the Certification Board of Infection Control (CBIC) or actively seeking certification. Knowledge of computer software/systems (i.e., word processing, spreadsheet and database programs). Demonstrates ability to exercise judgment to plan, execute and evaluate a program of actions to achieve infection prevention and control goals.. Required Licenses: Certn Board of Infection Control

　　Masters Degree In Nursing, Public Health or Allied Field. Membership in the Association for Professional in Infection Prevention and Epidemiology Inc. Solid body of knowledge of microbiology, epidemiology, principles of adult education.
