　　Our Housing Success Case Manager position provides an opportunity to work in a mission driven organization where your continuous work efforts are contributing to the greater good of the community. In this position, a detail-oriented individual will utilize case management techniques to assist formerly homeless individuals and families to increase their financial stability, improve their wellness, and reach greater self-determination.

　　At Housing Opportunities of SW Washington (HOSWWA) we work closely with local government and community partners throughout Cowlitz, Wahkiakum, Lewis, and Pacific counties to address issues of affordable housing and homelessness and to help families break the cycle of poverty. We provide affordable housing and housing assistance to low-income residents within our service area. The Housing Success Case Manager provides services to the agency by supporting clients in maintaining their housing stability.

　　Your skills and experience will support our mission as you:  

　　Conduct assessments and interviews with clients to determine eligibility.Engage residents to collaboratively develop goals and individualized service plans to support their housing stability, mental health, substance abuse, health, education, income, employment, and other priority areas.Support residents to maintain housing stability through individually tailored services and by facilitating effective connections to community services and resources.Guide clients through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed.Provide specialized and targeted interventions when clients are at risk of eviction.Connect clients to medical, mental health, and other supportive counseling services.Establish a network of community services and conduct appropriate referrals to clients.Track and report outcomes, providing data updates and reports as directed.Pay and Benefits

　　The starting pay range is $24.77 - $27.88 / hour. The full pay range is $24.77 - $34.28 / hour. Compensation for the position is negotiable, depending on the qualifications and experience of the chosen candidate. The final candidate for this position will be paid within the starting range.HOSWWA offers employees and their eligible family members the option to enroll in medical, dental, vision, and/or supplemental life insurance. Employees are also covered by Basic Life and Accidental Death and Dismemberment and Long-Term Disability insurance.All HOSWWA employees are members of the Public Employees Retirement System (PERS) administered by the Washington State Department of Retirement Systems (DRS). 　　High School diploma or G.E.D.

　　Preference for bachelor's degree in a social services field

　　Preference to persons with experience in case management, social services, or related fields.

　　Knowledge of case management methods and commitment to stay current on all technical information available in the field.

　　Working knowledge of region's social service systems and ability to establish a working relationship with local service providers.
