The HousingSpecialist focuses on navigating the Milwaukee County housing system andadvocating for the housing needs of the people enrolled in CCS.Essential Functions: Act as a member of the CCS team providing servicesto people with mental health, substance use, and co-occurring needs.Provide comprehensive housing services to people enrolled in CCSincluding locating housing resources, navigating housing system inMilwaukee County, and advocating for housing needs. Build off theperson\'s strengths to meet their individualized recovery needs.Allservices will be person centered and directed by the individual beingserved. Utilize various methods to serve people; including, but notlimited to: role modeling; monitoring; mentoring; supervision;assistance; and, cueing. Provide services in the community (includingpeople\'s homes) and other community locations. Participate as a teammember in recovery team meetings with the individual to createindividual recovery plans, identify and prioritize goals, and monitorprogress toward meeting identified needs. Appropriately document allcontact with the people being served and team members to adhere toprogram requirements. Responsible for accurate documentation andmaintaining a case file as required by agency policy, governmentregulations, and applicable local, state, and federal codes. Attend andparticipate in staff meetings, in-service training, seminars, andconferences as required. Responsible for maintaining flexibility indaily work schedule to adequately meet program needs. Responsible foradhering to agency policies and procedures while carrying out dailyresponsibilities.
