　　Hospice Aide / CNA Responsibilities:

　　Follow the interdisciplinary plan of care, be alert to thepatient\'s needs and concerns. Provide personal care, includingbathing, dressing, grooming, hair, nail, skin and foot care and oralhygiene. Assist patients, according to the plan of care.Perform simple procedures as an extension of therapeutic services,in accordance with the interdisciplinary plan of careObserve and report changes in the patient\'s condition and needs tothe Team Director (TD) or Registered Nurse (RN)May perform simple homemaking chores for the patient directly, mealpreparation, and/or light housekeeping in the patient\'s immediateareaComplete required documentation per policy. Participate incontinuing education programs. Follow confidentiality regardingpatients and families per policyDemonstrate a compassionate attitude toward patients and familiesconsistent with the philosophy and mission of the organization.Attend patient care conferences as requested by the TD
