Health Care Professional- Patient Navigator

Job Summary:
The patient navigator's primary role will be to assist appointment scheduling and access and to coordinate diabetes care for families who are at high risk of diabetes complications.
Key Responsibilities:
Identify families in need of patient navigator outreach
Outreach to the families via phone, email, portal messages, and in person in the clinic
Schedule patient visits
Set up Medicaid ride services
Refer diabetes management questions to the diabetes educator
Work Location:
On site – this role is expected to work onsite and is located in Aurora, Colorado

Minimum Qualifications:
Entry Level:
A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institutionIntermediate Level:
A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution
1 year of professional level experience working in quality improvement within a medical practice.
Senior Level:
A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution
2 year of professional level experience working in quality improvement within a medical practice.
Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.
Applicants must meet minimum qualifications at the time of hire.
Preferred Qualifications:
Master of Public Health (MPH) or related advance degree preferred
Experience in quality improvement in a type 1 diabetes clinic or clinical research environment.
Experience building and running reports in an electronic medical record, preferably the Epic system
Advanced knowledge of the Microsoft Office package including Word, Excel, PowerPoint, Access, and Outlook
Basic understanding of statistical and data analysis

Anticipated Pay Range:
The starting salary range ( or hiring range ) for this position has been established as
Entry: $51,788-$65,875
Intermediate: $56,358-$71,688
Senior: $60,928-$77,500
The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation depending on the level.
