Job Description
Position Summary:Lead instructor for assigned unit
Responsibilities:
Designs and implement individual program plans, daily activity schedules and community integration
Participate in training and directing Assistant Habilitation Specialists
Responsible for ensuring that all unit activities enhance and support each person’s social responsibility, independence, dignity and continued growth
Participate in all Interdisciplinary Team Conferences for all assigned participants either at the day habilitation program or residence
Act as the liaison between the two programs, ensuring that all information is forwarded to the appropriate person(s)
Complete all documentation and required paperwork including Annual and Semi-Annual day habilitation and staff actions plans, monthly review of goals and data, within specified time frames
Responsible for planning, writing, and implementing goals designed to maximize each participant’s level of independence, as determined by the IDT
Monitoring of participant’s progress by collecting, documenting, and analyzing performance data
Required to display proficiency while entering person served data documentation in the electronic data system
Establish and implement appropriate activity schedules for each individual
Communicate with residences and/or families regarding progress, incidents, etc
Act as a liaison for all community contacts and program staff
Serve as a positive role model for participants and staff members in a variety of program and community settings
Provide behavioral interventions when necessary following the least restrictive OPWDD approved techniques required by the situation
Ensure that the unit is clean, safe, stimulating, and appropriately decorated at all times
Participate and attend meetings/training at various sites
Participate in all routine program functions such as fire drills, mealtimes, arrivals and departures
Must be able to physically interact with the program participants, negotiate the environment independently, including assisting in all SCIP interventions, emergency CPR and or first aid techniques and fire drills
Maintain confidentiality at all times
Qualifications
Qualifications:
Associates degree required in Education, Human Services, or related field
Valid NYS driver’s license and two years’ experience working with people with developmental disabilities
