As an Intellectual Disabilities/Autism (ID/A) Habilitation Aide Specialist you will provide direct support services for our clients in their homes or in the community setting. 　　What are the required qualifications?

　　High school diploma or GED equivalent .

　　A valid driver's license with proof of car insurance, good driving record, and a reliable vehicle.

　　Ability to lift a minimum of 25 lbs.

　　Complete necessary documentation for each service provided, collect, and evaluate any applicable data and progress, and assess and further work with the team to develop positive strategies of support 

　　Provide direct support in accordance with the plan and ODP guidelines

　　Provide supports to children and adults in home and community-based settings to acquire, maintain and improve skills necessary to live in the community, to live more independently, and to participate meaningfully in community life.

　　Provide assistance, support, and guidance with self-care, maintaining health and well-being, home management, communication, mobility, relationship development and socialization, participation in community activities

　　Provide transportation for consumers to be able to attend engagement activities within the community, such as, but not limited to, community events, social appointments, errands, and volunteer activities.
