  Job Title: Family NavigatorGrade: 24Salary: $87,405 - $105,485The Research Foundation for Mental Hygiene seeks an organized, detail-oriented person to be responsible for care coordination and follow-up for families of infants with rare genetic conditions that put them at risk of autism spectrum disorder and related neurodevelopmental problems. The Family Navigator must be fluent in both Spanish and English. The incumbent candidate will work with research participants/patients in the Prospective Genetic Risk Evaluation and Assessment (PROGRESS) Autism Center of Excellence at Columbia University, in collaboration with the Research Foundation for Mental Hygiene and the NYP Center for Autism and the Developing Brain. The incumbent will act as a liaison between research participants/patients, researchers, and clinical and educational providers. The primary responsibility of the family navigator will be to link families of at-risk infants with appropriate services to help with early developmental needs.Duties and Responsibilities:• Facilitate care coordination across disciplines (e.g., physical therapy, speech therapy, early childhood education) and provide follow-up for patients/families in the community (e.g., engage with community-based providers (early intervention), attend meetings, help navigate applications for external services (insurance- or early-intervention based), and access external resources). • Work closely with Spanish and English speaking families to integrate care across the lifespan and bridge families to services in their communities to ensure their needs are being met both during their participation in our research program and from 2 to 36 months of age.• Partner with organizations and service providers to build relationships and connect patients to service providers across settings (e.g., in-home therapists, center-based early childhood providers or educators, and specialty clinic providers).• Facilitate continued maintenance and expansion of the resource database, including creating new contacts via outreach calls and in-person visits to learn about new resources for participants/patients. • Serve as a representative at community events and activities.• Maintain HIPAA-compliant records and protect personal information.• Perform other related duties as assigned.Minimum Qualifications:Bachelor's Degree in relevant field or the equivalent in training and experience plus 1-2 years of relevant work experience.
