　　Summary:

　　Responsible for assessing, educating, and making a decision regarding the nutrition therapy of patients.

　　Responsible for assuring that nutrition care is provided and documented in accordance with policies and procedures as well as regulatory guidelines.

　　Position Responsibilities:

　　• Provide nutritional assessment, education, counseling and documentation.

　　• Collaborates with medical professionals to assure patient nutrition needs are met.

　　• Attends medical rounds on units where applicable.

　　• Acts as resource for Healthcare Team as it relates to the patient.

　　Required Education:

　　Bachelor of Science Degree in Nutrition/Dietetics.

　　Training/Certifications/Licensure:

　　Valid registration with the Commission on Dietetic Registration (RD) or (RDE).
