Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provisionof food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.
Columbus House, along with its core of loyal supporters, friends and volunteers, has remained committed to fulfilling its mission: "To serve people who are homeless or at risk of becoming homeless, by providing shelter and housing and by fostering their personal growth and independence."
PurposeHouse and assist individuals in maintaining housing in the greater New Haven area who have been frequent users of jails and shelters via a multi-agency collaborative including DMHAS, DSS, CSSD, DOC, OPM, CCEH and Corp. for Supportive Housing.

Duties and ResponsibilitiesOutreach eligible clients and assist them with completing and submitting HAF/PHA/DSS housing applications; assess needs and strengths to develop clients goals.Assist with identification and documentation for securing Basic Needs and entitlements such as Medicaid, Supplemental SS, SSDI, Nutrition Assistance, housing subsidies etc.Help identify, secure, and occupy safe and affordable housing.Assist with budgeting, bill-paying, housekeeping, shopping, and cooking.Help integrate mental health and addiction treatment and use of medical and wellness services.Encourage social supports such as peer activities and groups; develop connections to the community.Connect tenant to job services and employment alternatives such as volunteer work or steps towards achieving employability such as a drivers license or completing vocational training.Other related duties as assigned.##
Qualifications, Knowledge, Skills and AbilitiesBachelors or associate degree with 3 yrs. relevant work experience.Drivers License, good driving record and excellent writing skills.Able to effectively use the network of substance abuse, mental health, and social services providers.Familiar with criminal justice; able to engage, manage, motivate, and support.Time management skills and computer literate with a deadline-oriented mindset.Able to meet documentation requirements, complete knowledge base of all aspects of the delivery of person-centered case management.Able to assess strengths, barriers, and needs to assist in identifying personal goals, values, and priorities.Ability to advocate for clients as well as support self-advocacy.Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; capacity to work within a team and foster teamwork.Flexible, able to seek out opportunities for clients to lead fulfilling lives.Crisis intervention skills, boundaries, confidentiality, universal precautions, harm reduction, strong empathetic skills using the Trauma Informed Care Values:SAFETY, TRUSTWORTHINESS, CHOICE, COLLABORATION, and EMPOWERMENT.Ability to maintain own wellness and continually strive for improvement.Supervised by:Supportive Housing Manager
Salary:$19.23-$21.23 per hourplus excellent benefits
Schedule:40 Hours per week,Monday - Friday 8:00 am to 4:00 pm
