Position Overview: The Consumer Directed Care (CDC) Skills Trainer provides program overview to interested consumers and their surrogates.
Position Responsibilities: Responsible for intake and orientation for new CDC referrals and determining eligibility; carry a caseload of consumers who are enrolled in the CDC service model. This position will also be responsible for assisting with marketing the CDC service model in and round the Greater Lynn area. This included providing presentations to small groups of people, as well as individual presentations and other activities specific to growing the model as needed.
Qualifications •Minimum of a Bachelor's Degree in Human Services with two (2) years of professional experience in the care of persons over the age of sixty-five (65) required•Knowledge of Community Resources preferred•Ability to make proper judgments, demonstrated leadership qualities, good written and verbal skills, and ability to maintain effective interpersonal relationships with staff members, consumers and partners•Possess the knowledge and skills necessary to work successfully in the interdisciplinary team and to provide or arrange for services•Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook •Ability to process information accurately •Ability to work both independently and as part of a team•Professional demeanor and ability to handle confidential materials with discretion•Must have reliable transportation and valid driver's license for community outreach and home visits•Bilingual - Spanish speaking required
Hours: Full Time - 40 hours Monday through Friday
