Description

The Community Health Worker II (CHW-II) is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-II will provide job duties within at least one of the following tracks:

Required qualifications:

2 years Experience working and/or volunteering in the community highly preferred.
