　　Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

　　Required Qualifications:

　　Bachelor's Degree in Nursing or related field (if Associate Degree in Nursing) from a nationally accredited program

　　Master's Degree iin Health-related or Business Administration

　　Upon hire: Washington Registered Nurse License

　　Within 1 year of hire Accredited Case Manager (ACM) or Commission for Case Manager (CCM)

　　Within 2 years of hire Professional Development Certificate (master's degree in education or nursing education can be in other related areas

　　8 years Relevant experience in Case Management (Care Coordination or Utilization Management)

　　8 years Progressive leadership roles within Care Management

　　Preferred Qualifications:
