Summary TheClient Navigator is primarily responsible for the support and educationof end-users, caregivers, and payors with Atos products and services inthe Case Management Program. The Client Navigator works in collaborationwith the Atos Customer Support/Advocate team inquiries and concerns toassess, facilitate, plan and coordinate integrated product access forthe end user. The navigator will develop and implement an individualizedcare plan in collaboration with the patient or patient\'s supportnetwork to address the patient\'s specific product needs and utilizationto reach mutually established goals over time. The Navigator will workto ensure that the patient is able to access the products they need tomake progress toward their desired outcomes. Essential FunctionsDevelops and implements an individualized case management plan incollaboration with the patient, caregiver, physician and/or otherappropriate healthcare professionals and member\'s support network toaddress the member needs and goals and to ensure product access. Partnerwith other entities involved in the member\'s care, including medicalproviders, therapists, payor case management programs as well ascommunity-based organization, in an effort to promote self-managementand a high quality of life post laryngectomy. Maintains ongoing membercase load for regular outreach and management. Promotes utilization ofproducts and services available for patients. Applies principles ofself-management and motivational interviewing as well as Atos clinicalguideposts to educate, support and motivate change during membercontacts.
