About the Department
The Pediatric and Adolescent HIV/AIDS Program, Care2Prevent, in thesection of Pediatric Infectious Diseases provides comprehensivetreatment and prevention services for people living with HIV and thosewho are most vulnerable to HIV infection. Job Summary
The Client Care Liaison manages communications and scheduling withCare2Prevent clients to support their care. They provide excellentcustomer service and will help coordinate with various members of thecare team, help maintain appointment records, develop reports onappointment attendance, and triage client communications for action.Through their work, they will also help to develop systems and processesfor managing appointments and appointment communications. The ClientCare Liaison is an essential member of the Care2Prevent team,interacting with a multidisciplinary team to ensure seamlesscommunication between clients and care providers with an emphasis onclient care engagement and appointment adherence.
This at-will position is wholly funded by contractual grant fundingwhich is renewed under provisions set by the grantor of the contract.Employment will be contingent upon the continued receipt of these grantfunds and satisfactory job performance.
Responsibilities
Support client appointment scheduling and develop reports to trackappointment attendance.Manage appointment reminder communications and follow-up.Interface with the clinic referrals desk to support clientappointment scheduling in the C2P clinic.Help support the team in completing new client intake such asrecording eligibility documentation and collecting contact anddemographic information.Oversee the C2P clinic schedule including actively participating inpre-clinic huddles to provide updates on clinic schedule changes.May initiate correspondence with clients and other written materialsunder own signature or for signature of the leadership team.Triages emails and telephone calls for action. Resolves mostlyroutine and some complex inquiries.Prepare special reports and summaries, assists in managing dailyoperation of the office, and may assist in developing administrativegoals and policies.Performs other related work as needed.Minimum Qualifications
Education:
Minimum requirements include vocational training, apprenticeships or theequivalent experience in related field (not typically required to have afour-year degree).
