Responsible for the integration of all resident services provided by the Adult Care Facility into a whole system that maintains a continuing awareness of each resident's total well-being. Essential Duties & Responsibilities: *Initial & periodic evaluation of needs of resident and capability of facility program to meet those needs. *Responsible for admissions and Discharges including documentation & communication. *Psychological/Social Assessments. *Development of individualized service plans: Daily monitoring for changes and reviews. *Orienting new residents and families to daily routine and services. *Assisting each resident to adjust to life in the facility and assist and maintain family and community ties. *Assist residents in obtaining and maintaining a primary physician or source of resident health and mental health needs. *Assist residents in making arrangements to obtain services, examination and reports needed to maintain or document the maintenance of the resident health or mental health and facilitate transportation. *Assisting residents in need of discharge planning to make and execute sound discharge or transfer plans. *Assist in the establishment and operation of a system to enable residents to participate in planning for change or improvement in facility operations, programs, grievances and recommendations. *Assisting with and arranging for services from public and private sources. *Assisting residents in making application for and maintaining income entitlements and public benefits. Basic Duties & Responsibilities: *Provide information and referral. *Maintain confidentiality and current resident files and notes. *Maintain Quality Assurance activities. *All other duties as assigned. *Adheres to all fire safety & evacuation procedures. *Coordinate the work of other service providers within the facility.
