This is a remote position, but candidates must live in Minnesota How Is This Role Important to Our Work? The Behavioral Health (BH) Peer Navigator is a critical component of the BCBSMN Care Management team who supports and advocates for members or their loved ones who have been diagnosed with a mental health or substance use condition. The BH Peer Navigator role helps members discover their strengths and develop unique recovery goals. These services aim to assist individuals experiencing acute mental health or substance use challenges by providing culturally sensitive support, resources and guidance! A Day in the Life: Receives referrals, outreaches both telephonically and in-person to member or designated representative and providers to engage in the care management program. Leverages expertise to advocate for member needs, design plan of care, and assist member in next steps. Collaborates with care management team, consults where pertinent, & communicates with member to help with recovery goals. Follows up with member after transitions of care to assist with member education & presenting needs. Serves as a member advocate by identifying underlying needs and facilitates referrals to external resources and services in the health care system, as appropriate. Assists in reducing barriers to community resources, provides encouragement for involvement in community supports that support recovery goals and interests. Proactively provide information and education to customers, as appropriate, about a variety of health, financial, and self- service programs. Conducts outreach to and engages members in case management programs. Works with member through the program duration and assist with non-clinical care plan goals. Coordinates care with the recipients care team Shares personal stories of resilience & recovery where applicable with members experiencing behavioral health conditions Ensures member data is documented according to BCBSMN application protocol and regulatory standards. Maintains a strict standard of confidentiality to ensure that our members protected health information (PHI) is secure. Required Skills and Experiences: 3+ years of related experience. All relevant experience including work, education, transferable skills, and military experience will be considered. Has a lived experience of mental illness or substance use, is trained to provide direct mental health services and has a passion for supporting others. Level II Certified Peer Specialist certification which meets all requirements of a level I Certified Peer Specialist (CPS) and is a qualified mental health practitioner per DHS requirements or Certified Peer Recovery Specialist Reciprocal (CPRSR). Strong interpersonal communication skills such as: active listening, validation, empathy, & open-ended questioning. Nice to Have: Relevant 1+ years managed care experience, e.g. case management/health coach, utilization management and/or auditing experience. Experience working in a health insurance company NCQA or other regulatory familiarity. Lived experience as a part of one of the following marginalized communities: Indigenous American, Black/ African American, or Hispanic/ Latino.
