Description
As a Case Specialist, you will play a pivotal role in providing compassionate and comprehensive case management services to our clients. We are seeking a fluent speaker of Dari or Farsi to effectively communicate with individuals from diverse linguistic backgrounds. Your primary responsibilities will include reviewing and processing case files, assisting participants with necessary services and housing referrals, and ensuring accurate and up-to-date recordkeeping. You will collaborate closely with other team members, including supervisors, to evaluate cases, make informed decisions, and provide support to those in need.
Responsibilities:
Case File Review: Receive and review applications from individuals seeking assistance, ensuring completeness and accuracy. Analyze cases to determine eligibility and suitability for support based on established criteria.
Language Proficiency: Utilize fluency in Dari or Farsi to effectively communicate with clients who may have limited English proficiency. Facilitate clear and accurate information exchange during client interactions.
Case Management Services: Provide comprehensive case management services to clients, including assistance with housing resources and referrals to relevant support services.
Customer Service: Interact with clients in a professional and compassionate manner, addressing their inquiries, concerns, and needs with empathy and sensitivity.
Social Services Liaison: Collaborate with social services agencies, welfare offices, and workers to ensure clients receive appropriate support and resources.
Case File Maintenance: Ensure accurate and up-to-date maintenance of case files, documenting all client interactions, progress, and outcomes. Regularly review case files for completeness and consistency.
Supervisory Collaboration: Work closely with supervisors to review and assess cases, seeking guidance when necessary, and contributing to the decision-making process.
Training and Development: Engage in ongoing training and professional development opportunities to enhance case management skills and stay updated on relevant policies and procedures.
Approvals and Funding Requests: Facilitate the approval process for funding requests, ensuring compliance with organizational guidelines and funding availability.
Team Collaboration: Collaborate with fellow case specialists and team members to share insights, knowledge, and best practices for providing exceptional client services.
Requirements
Qualifications:
Fluent in Dari or Farsi (written and spoken) is a requirement for this position.
Previous experience in case management or a related field is preferred.
Knowledge of social services and experience working with individuals with disabilities is beneficial.
Familiarity with former PCG employees or ResCare is advantageous but not mandatory.
Strong customer service skills with the ability to interact effectively with diverse individuals.
Excellent organizational and time management skills to manage case files efficiently.
Detail-oriented and able to follow processes accurately to ensure compliance.
Empathetic, compassionate, and patient approach when dealing with clients.
Proficiency in MS Office and other relevant software for case file maintenance.
Ability to work both independently and collaboratively within a team environment.
