Please see our positions listed below.Case Manager, LEAD Program, Walla Walla & DaytonThis position coordinates with other case managers, an outreach manager, and supervisors on the Law Enforcement-Assisted Diversion (LEAD) Case Management team, supporting people with behavioral health issues in Walla Walla and Columbia Counties. The case manager will also closely liaise with mental health providers, substance use treatment staff, and outside partners in behavioral health. Starting salary range for this position is $52,000--$56,000 gross compensation per year, plus BMHTH benefits listed above.
Duties:
Understand and adhere to national LEAD Model guidance for case management and trauma-informed care practicesEnsure confidentiality of all Blue Mountain Heart to Heart (BMHTH) clients, current and formerProvide intensive case management and referral services for health care needs, social safety net services, and other appropriate care as provided by BMHTH and partnering agenciesProvide timely, accurate, and complete reporting on cases in Julota or other health records systems, subject to regular review by supervisory staff, funding organization staff, and/or Executive Director or their designee.Offer wraparound services to clients for housing support, food and nutritional assistance, workforce re-entry assistance, mental health support, substance use treatment referrals, and other support as neededLiaise regularly with program stakeholders on the Operations Work Group, and communicate with them regarding client progressAttend LEAD case management training as requested and required by funding organizationsExpectations:
