This Case Manager position supports a diverse range of individuals in a site-based housing program for Avivo Village. The Case Manager will work in programs deeply rooted in Harm Reduction and Housing First practices as part of Avivo's Ending Homelessness Division's continuum of care. The position supports single adults who have previously experienced long-term or chronic homelessness, many of whom are people with an active substance use disorder. The Case Manager will actively support residents in achieving housing stability through harm reduction and assertive and progressive engagement approaches to support individuals in achieving their personal goals. The Case Manager understands safe housing for all is a social justice issue founded in the understanding that housing is an intrinsic human right. The Case Manager will be working in an unpredictable work environment and is highly skilled in meeting people where they are at, while delivering the highest quality services using culturally responsive and evidence-based practices in a trauma-informed environment. PRIMARY RESPONSIBILITIES: Engage and build rapport with clients. Assess eligibility for program and services and connect clients. Develop Goal Plan. Make appropriate referrals, monitor progress, and ensure follow-up. Utilize creative approaches to meet in person, regularly with clients and address their needs. Immediately consult about any safety related concerns (for client and staff), mandated reporting concerns, and complaints from the client. Provide housing specific services and build landlord relations. Paperwork/Documentation/Data/Compliance completed on time. Develop Crisis Plan and conduct crisis intervention as needed. Ensure services provided are culturally responsive utilizing best practices. Qualifications High School Diploma or GED required. One or more years of experience working with single adults experiencing homelessness or similar experience. Lived experience will be valued and considered as part of the experience requirement. Must have a valid, current driver's license, access to a personal vehicle, proof of valid insurance and a clean driving record. Mileage reimbursement will be provided. Able to pass DHS background check.
