Description

Robert Half is currently seeking a highly motivated Mental and Behavioral Health Case Manager for a prestigious client in the healthcare industry. The ideal candidate will have comprehensive knowledge of case management principles and a genuine interest in helping individuals with mental health and behavioral issues reach their recovery goals.

Responsibilities:

· Provides comprehensive case management services including intake, assessment, crisis intervention, advocacy, and linkage to community resources.

· Collaborates with multidisciplinary team members to develop effective care plans and interventions.

· Monitors client progress toward established goals and adjusts care plans as needed.

· Provides client education on mental health, medications, coping mechanisms, and community resources.

· Assists clients with transitioning back into the community after institutional care.

· Maintains accurate and timely records conforming to regulatory standards.

· Performs duties within professional standards and ethical guidelines.

Requirements

· Bachelor's degree in Social Work, Counseling, or a related field required,

· Current and valid case management certification.

· Minimum 2 years' experience in mental health or behavioral health case management.

· Strong knowledge of community resources, treatment plans, and mental health law.

· Exceptional interpersonal communication skills, both verbal and written.

· High level of discretion and professional approach to handling confidential information.
