　　Provides clinical case management addressing life domains such as housing, finances, transportation, legal services, vocational, employment, healthcare, and family strengths/needs in Atlantic County. Develops care plan that is person centered and includes strategies for recovery. Works to coordinate with other treatment providers or behavioral health agencies individual is involved with. Provides linkages to appropriate care and resources in the community including resources for specialized needs such as HIV/AIDS, mental health disorders, chronic and acute health problems, and legal involvement. Demonstrates education/training, motivational skills and advocacy to refer and link individuals to SUD treatment and appropriate level of care transitions. Collaborates with program team and supervisor. Must be able to operate an agency vehicle and maintain a driving record acceptable to the agency.

　　Education, Skills, & Experience:

　　Bachelor’s degree in mental health discipline or Bachelor’s degree with NJ CADC, LCADC. Must have at least 2 years’ experience working with high need individuals with substances use disorders and/or homelessness population. Knowledge of formal/informal community systems and resources.
