Statement of Purpose: The Care Coordination Assistant is responsible to work with a multidisciplinary team to optimize the discharge process and help to support a safe transition of care that is primarily directed by RN Case Managers and Social Workers. Additionally, the CC Assistant will act as a liaison to skilled nursing facilities and will communicate with external facilities via the electronic discharge software program i.e. Allscripts, in writing and telephonically as directed by the Director, Coordinator, RN Case Managers and Social Workers. The assistant will prepare discharge referrals/paperwork, arrange post discharge transport as needed, help with facility/home care referrals, arrange certain follow up appointments, arrange for DME, assist with the delivery and documentation of regulatory forms (such as the IM and MOON), and perform all other duties as directed. In addition, the CC assistant will respond to insurance company inquiries telephonically
Qualifications: Education: Associates or Bachelor’s degree in healthcare or equivalent experience preferred . Experience: Previous work in healthcare environment required. Hospital based administrative experience preferred. Experience working in multidisciplinary professional environment. Knowledge of Microsoft Word and Excel required. Previous experience with Meditech preferred. Basic medical terminology knowledge required. Excellent customer service skills critical. Effective interpersonal skills essential.
