What we want
The Community Health Worker II, under general supervision, recruits patients for the clinical practice, assesses community health needs, and connects patients to social and community support resources. Assists in data collection and program assessment. Provides periodic support to outpatient clinic functions.
What you need to know
Compensation will be commensurate to selected hire’s experience.
Required Education and Experience:
Associate degree in applicable field or equivalent combination of education and experience.
Four years of experience in public relations, outreach, training, or related areas.
Required Licenses and Certifications:
Must have valid State of TX Driver’s license
Department of State Health Services (DSHS) certification, certified CHW and/or CHWI, Promotora Certification of Training, or ability to successfully complete Promotora Certification.
Required Knowledge, Skills, and Abilities:
Strong verbal and written communication skills in English and Spanish.
Ability to multitask and work cooperatively with others.
Ability to work with sensitive information and maintain confidentiality.
Strong interpersonal and organizational skills.
Knowledge of word processing, spreadsheet, and database applications.
Strong computer literacy including ability to troubleshoot and provide technological support.
Preferred Qualifications:
Two years of related experience as a Community Health Worker working with Spanish-speaking community members, stakeholders, and completing research and outreach activities.
Public relations and marketing experience.
Experience with assisting or leading training efforts.
Experience with data collection and data management efforts.
Knowledge of health care education.
Responsibilities:
Program Planning & Implementation: Participates in participant recruitment, program planning, implementation, and evaluation including managing appointments, developing program material, assisting support staff in the flow of participants and data collection. Participates in research and scholarly activities related to health promotion and disease prevention.
Program Events: Plans, organizes, and participates in community outreach events, health fairs, and community advisory board meetings. Connects patients to social and community support resources.
Program Supervision: Supervises, mentors, and coaches fellow Community Health Workers both in office and community settings.
Program Communication: Communicates with “Together for a Better Tomorrow” participants, serving as a liaison between program research staff, community members, and stakeholders.
Data Management: Manages program data to meet program and participant needs, including coordination and communication of program records. Performs other duties as assigned.
