  As a Social Work Case Manager - Behavioral Health, you need to know how to:Coordinate the care and services for patients identified as needing assistanceor meeting care management criteria.Assess the patient's prior level of functioning, access to and/or use ofcommunity resources and available support systems.Identify appropriate admission and continued stay issues.Attend, facilitate, and participate in rounds and case conferences.Conduct comprehensive behavioral health assessments for patients on mentalhealth hold in the ED and on medical floors when ordered by a physician.Participate in in multidisciplinary care planning meetings.Communicate and collaborate with medical, interdisciplinary team staff,patient, family, insurance companies, and social agencies to providequality discharge plan to appropriate, safe, level of care.Provide community resources for psychosocial, financial, and educationalneeds of patient and family.

But we also expect excellence, which is whywe require:Master's Degree in Social Work (MSW) from an accredited programSuccessful completion of Licensed Clinical Social Worker (LCSW) licenseexam and current clinical supervisionCurrent LCSW license or the ability to obtain within three (3) years of hireMinimum of two (2) years of clinical experience

  Preferred.At least two (2) years of case management experience in an acute care setting
