About Behavioral Health Discharge Planner

  Position Purpose: Manage the coordination, planning, and placement resolution for all inpatient admissions in the assigned service area. Work directly with the hospital staff, other agencies, Foster Care members, Medical Consenters, Caregivers, Department of Family and Protective Services (DFPS) to facilitate and coordinate access to behavioral health services and community resources and to ensure timely and clinically appropriate discharge plans are developed and implemented.

  Impact complex systems of care through coordination efforts to ensure timely placement of children who have stabilized at the inpatient level of care

  Educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices

  Conduct outreach and education with varied stakeholders including hospital clinical teams, DFPS, child-placing agencies and outpatient providers to facilitate and ensure the development of timely and appropriate discharge plans

  Appropriately document discharge planning outcomes and barriers; help identify and interpret any trends including systemic issues, access or placement needs and recidivism, and implement interventions to impact negative trends

  Assist DFPS with accessing current medical information as needed

  Facilitate access to behavioral health services, including assistance with locating providers and scheduling appointments as necessary

  Coordinate the sharing of health information between providers and other programs

  Document all interactions and coordination efforts with both internal and external customers in the Care

  Enhance Clinical Management Software (CCMS) online file system

  Education/Experience: Bachelor's degree in Social Work or related field or equivalent experience. 2+ years of related social work or behavioral health experience with solid clinical knowledge and experience with the treatment planning process. Experience with the state's Foster Care system preferred.

  Preferred Qualifications:

  Bachelor's degree in social work or behavioral science, or LBSW, and 2+ years of social work experience, patient advocacy, or coordinatizing medical and/or behavioral health services for Medicaid members.

  3+ years' of behavioral health experience in local or state mental health authorities, in-patient or out-patient treatment facilities, community-based mental health settings, and/or behavioral health managed care environments.

  Experience working directly with adults, children, and adolescents' members with mental health conditions or special needs.

  Experience collaborating with medical and behavioral health providers and managing community and mental health resources for STAR Plus and STAR Kids Medicaid recipients.

  Experience managing high volume caseloads.

  Role requires strong organization and customer services skills.

  Must be able to work independently with minimal supervision.

  Proficient computer skills and experience working within large databases or provider information systems and Microsoft Office applications.

  Pay Range: $25.44 - $45.76 per hour
