　　Patients and families near the end of life should have a caregiver committed to the art, science, and skill of delivering compassion.

　　Death is a meaningful experience for patients and loved ones. We only have one chance to get it right.

　　Delivering compassion the right way is not easy; it takes skill, heart, commitment and structure to do it well.

　　We believe the effort is worth the challenge because nobody should die alone.

　　Requirements:

　　Assumes overall responsibility for the medical component of the palliative client care program and supports the hospice program.

　　Be responsive to the medical community to ensure high-quality, appropriate, and efficient utilization of hospice services.

　　Assist in investigations of important problems in patient care and clinical performance.

　　Assist the Agency in the coordination of patient care between clinical staff and patients' attending physicians.

　　Acts as the Physician Representative of the interdisciplinary group (IDG) and/or attending physician to certify and recertify for subsequent election periods that the patient is terminally ill.

　　Current MD or DO

　　Legally authorized to practice medicine in the state of practice.

　　Availability:

　　4 hours every 2 weeks of in-person availability in the Green, Ohio office for IDT meetings.

　　On Call Responsibility shared with MD team to be discussed in interview
