Detailed Job Responsibilities: 45% Operation of document scanner: Review medical record documentation in order to collate, sort and review Perform scanning of documents into the Electronic Health Records Electronically file care-related medical record documents. 40% Abstraction: Independently abstract and enter patient information from paper medical records into the Electronic Medical Record - EMR. Abstract the following information from the paper medical record and enter into EMR: Point and Click: Allergies, Immunizations, Medications, Problem List, and Chronic Medical Conditions. 5% Verification: Verify completeness of data entered into the EMR. 5% Backup Faxes Downloading to chart Printing and abstracting information 5% Miscellaneous: Promote a safe, equitable, and respectful environment in which concerns can be addressed effectively. Other duties as assigned. Qualifications Required Qualifications: High School Diploma / GED and four years of office service experience. Training / education may be substituted for some of the years of experience. Thorough knowledge of and experience with medical terminology, anatomy, physiology, dosing and pharmaceuticals and data entry skills are essential in a clinical setting. Detailed oriented Must have a minimum of one year of experience in HIM Experienced in working with EHR Demonstrated commitment to promoting a culture that respects and advances diversity and inclusion. Preferred Qualifications: Previous electronic medical record experience. Previous experience in nursing, medical coding, health information or medical transcription in an outpatient clinic or acute care setting. Direct patient care experience. Attention to detail. Must have ability to accurately read medical reports and extract important and pertinent information from medical charts. Experience working with individuals from diverse racial, ethnic, and socioeconomic backgrounds.
