  Job purpose

  Fair Haven Community Health Care seeks to expand access to HIV prevention services across Greater New Haven, in addition to closing other gaps in care related to access to medications. The Care Coordinator (PrEP and Access to Medications) engages patients, coordinates care, and expands access to pre-exposure prophylaxis (PrEP) for HIV prevention for patients and community members. The Coordinator also coordinates other gaps-in-care initiatives related to medication access, including ensuring that appropriate patients receive evidence-based medications for chronic disease: e.g. aspirin and cholesterol-lowering medications. As a member of the Care Coordination team, the Coordinator also identifies and addresses health related social needs among patients and assists in closing other gaps in care. The Coordinator will serve as the key internal and community-facing coordinator for patients on or eligible for PrEP.  

  Duties and responsibilities

  Primary responsibilities include but are not limited to:

  Maintains a census of patients prescribed PrEP (oral and injectable formulations) at FHCHC and work with clinical teams to ensure that quality PrEP care, including laboratory monitoring, is occurring on a timely basis.

  Maintains a census of patients requiring statins for prevention of heart disease and works with clinical teams to ensure that prescriptions are appropriate and timely.

  Maintains a census of patients requiring aspirin for management of vascular disease and works with clinical teams to ensure that prescriptions are appropriate and timely.

  Maintains a census of patients on antipsychotic medications and ensures that yearly metabolic monitoring occurs.

  Responds to referrals from providers across FHCHC sites for PrEP case management services.

  Liaisons with external partners, particularly the New Haven Pride Center and A Place to Nourish Your Health, to accept referrals on a timely basis (within 30 days).

  Works with Grand Pharmacy staff to ensure patients benefit from its services and payment opportunities.

  Screens patients for health-related social needs and works to address those needs through community linkages and other services.

  Assists patients in their applications for pharmaceutical assistance programs for PrEP.

  Participates in education efforts targeting the FHCHC and external community regarding PrEP eligibility and effectiveness.

  Ensures full compliance with HIPAA by protecting all personally identifying information associated with a patient.

  Collaborates closely with Population Health team at Fair Haven, along with physicians, nurses, and medical assistants to ensure that patients have integrated, comprehensive and coordinated care.

  Attends regular staff trainings and meetings as needed to ensure a fluid and effective team-based approach to patient-centered care.

  Works with FHCHC’s Data Team to prepare grant-related reports pertinent to HIV prevention.

  Works with FHCHC’s Data Team to maintain and act on lists of patients eligible for PrEP based on risk factors.

  Qualifications

  Associate’s degree required, Bachelor’s degree preferred. Significant experience in community or public health may substitute degree requirement.

  Bilingual (English/Spanish) is required.

  The selected candidate will have excellent interpersonal and organizational skills and be comfortable with learning a new computer system (Fair Haven’s electronic medical record system is Epic).

  Commitment to serve LGBTQ and minoritized populations, regardless of immigration status and ability to pay, with humility and compassion.

  Community Health Worker core competency training is a plus but not required.

  Documented experience with Microsoft Excel.
