Description:
Full-Time Triage RN working in the Albany Hospice Office -
The Community Hospice has a full-time rewarding opportunity for a Triage Hospice RN to meet the needs of Hospice patients and families. The work schedule is Monday-Friday 8a-4p and a rotation requirement of every other weekend for a total of 40 hours per week. This position is ideal for those that have the flexibility to work with a variety of patients as needed without the responsibility of a caseload.

What you will do:
Meet the needs of Hospice patients and families by responding to calls , triaging to handle caller needs via phone when possible and assigning visit RNs to see patients as appropriate
Make calls to patients/families proactively as assigned (checking in).
Hours worked Monday-Friday will be at the Community Hospice office -whereas hours worked on the weekend will be performed remotely.
What you will need:
A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
2 years+ recent experience as an RN in acute care or Hospice/Home Caren preferred
Valid Driver's License and Reliable vehicle
Ability to actively listen and respond appropriately to patients/families as well as to support patient/families through times of crisis.
Ability to learn/use an electronic medical record
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
Understanding of Hospice principles preferred
Pay Range: $35.00-$47.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
