About the Role:
The Transition Specialist builds and fosters relationships withcommunity partners (i.e. Rutland Regional Medical Center, lawenforcement, schools) around transition needs. The Transition Specialistconducts training, transition and crisis planning, and step up/step downplanning. In addition, this position assists CCN teams to ensureeligibility status is determined, both clinically and financially. Thisposition is a liaison for new and existing families, individuals andteams until long term supports are engaged and maintained.
Principal Responsibilities:
- Acts as an interim contact when an eligible individual is waiting forpermanent supports to be in place.
- Develops transition plans and ensures Developmental Service staff areadhering to timeframes and supports are started.
- Establishes transition plans and placement relationships for childrenaging out of Department of Children and Families custody so there is awarm, well considered transition.
- Assists high needs individuals to establish guardianship, payeeservices and social security prior to adult Developmental services.
- Assists individuals not receiving Home and Community Based Servicesto assess career goals and best fits.
- Acts as a temporary support for individuals moving from Bridge, orother internal CCN teams while they wait for permanent supports to be inplace.
Qualifications:
- Bachelor\'s degree in related field preferred; equivalent experiencemay be considered.
- Previous experience in supportive employments required.
- Previous experience working with school\'s special educationdepartment desired.
- Previous work with crisis triage and engaging teams to build supportsin step up/step down scenarios.
- Proficiency with computers, experience entering data into electronicmedical records (EMR) program.
- Ability to work with others in a positive, responsible manner.
- Strong writing and organizational skills in order to receive andconvey information in efficient and effective manner.
- Flexibility, dependability, and ability to work independently.
- Able to flex schedule as needed.
- Must have a valid driver\'s license, reliable transportation withpersonal automobile liability insurance coverage at or above the levelsmandated by the State of Vermont.
