  Job Description/Essential Functions:

  Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

  Currently, we are seeking a full time Tenant Service Coordinator to work in our Behavioral Health Services Division. This position has the responsibilities of coordinating communication and issue resolution between Services Provider, Individual Tenant and related Landlord, and the Clearinghouse and serves as point of contact for Clearinghouse Tenant Services Liaison (TSL) for all tenants served by the provider agency.

  Some other responsibilities include the following:

  Serving as point of contact for the Tenant Services Liaison (TSL) for all tenants assigned to the lead Agency by Community Services Staff (CSS). Provide tenant service coordination to individuals moving from congregate living homes to Permanent Supportive Housing as well as to individuals in approved Homeless Programs.Performing liaison responsibilities for a minimum of 50 tenants and a maximum of 100 tenants.Coordinating communication and problem resolution between individual tenant and/or prospective tenants, related landlord, Clearinghouse/Tenant services Liaison (TSL), and recovery support service provider.Developing and maintaining tracking mechanism for information as required by TSL and Lead Agency administration. Generate monthly feedback reports from case manager or designated staff point of contact.Providing orientation and training to agency staff and tenants on issues including Tenant Rights, Terms of Lease, Fair Housing and Requirements for Successful Tenancy.Communicating with the CSS staff regarding level of care, concurrent and clinical reviews and relevant changes/amendments. This includes attendance at weekly agency specific CSS meetings.Ensuring tenant files, including the housing application and appropriate and related documentation and verifications, are reviewed for accuracy/completion, submitted timely and maintained.Performing outreach to prospective tenants to ensure they can be reached quickly when a unit is coming available.

  Job Qualifications:

  Bachelor's degree in Human Services/Social Services, Healthcare, or a related field and 3 years of residential and or supported housing experience preferred or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.Experience with individuals who have severe and persistent mental illness, addiction and other challengesKnowledge of recovery oriented principles, Affordable Housing and Tenants Rights Practice Guidelines and supports.Excellent customer service and problem solving skillsStrong interpersonal communication and analytical skills and ability to mediateAbility to work independently and as part of a focused team.Proficient with the use of office equipment (e.g., fax, photocopier machine, scanner, pager, shredder, telephone, computer, etc.)Possession of a valid driver's license with an acceptable driving record.Scope:Frequent ta king and listening.Possess manual dexterity and fine motor skills.Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with clients.May require crisis intervention in emergency situations.Flexibility in scheduling and available to do 24 hour on-call coverage.Local travel required, including transporting program participants.May be required to report to work during emergencies, including inclement weather.
