  The Division of Metabolism, Endocrinology and Nutrition, Department of Medicine, University of Washington is accepting applications for one (1) part-time Advanced Practice Provider (APP) at the rank of Teaching Associate with a .40 FTE. As a member of the Glycemic Control Consult Service, the APP will provide consultation to medical teams, patients and families, and nursing staff. The APP is responsible for providing Glycemic Control Consult Services independently and collaboratively with appropriate privileges, including initial and ongoing assessment, diagnosis and treatment of patient’s medical, physical and psychosocial status.

  STANDARDS OF PRACTICE AND PERFORMANCE1. Provides timely initial patient and family assessment for Glycemic Control Consult Service requests.2. Attends Glycemic Control Consult Service meetings with attending physician and in collaboration with the treating medical team.3. Facilitates surveillance activities to proactively offer consultative services to primary teams.4. Participates in the assessment, planning, implementation, and evaluation of Glycemic Control Consult Service interventions.5. Collaborates with the attending physician, PCP, and other providers to ensure coordination and continuity of care across settings and over time.6. Attends appropriate clinical meetings to serve as a resource for staff and providers.7. Facilitates collaboration with the Outpatient Diabetes Care Clinic to ensure continuity of care.8. Provides patient and/or family with health education, counseling and referrals to other health professionals and community resources as appropriate.9. Participates as a key member of the interdisciplinary team through the development of collaborative and innovative practices and evaluation of patient care outcomes.10. Evaluates quality outcomes for the Glycemic Control Consult Service through participation in the development of quality indicators.11. Participates in the assessment, planning, implementation of transition care, and evaluation of the following categories of patients:o Established patients recently discharged from the Glycemic Control Consult Service to the outpatient clinic settingo Established patients at the University of Washington School of Medicine Diabetes Instituteo Transplant patients and hyperglycemic patients requiring urgent care at the University of Washington School of Medicine Diabetes InstituteGLYCEMIC CONTROL CONSULT SERVICES APP DUTIES1. The Glycemic Control Consult Service APP maintains excellent interpersonal skills to execute the UWMC Glycemic Control Consult Service Best Practice Model.o Establishes a therapeutic relationship by imparting a commitment to care for the patient and family through the acute illness and beyond.o Connects with the patient and family by eliciting the patient/family perspective and validating the value of that perspective using strong empathic listening skills.o Communicates a sense of caring and openness.o Communicates effectively with patients and families, appreciating cultural and socioeconomic disparities.2. The Glycemic Control Consult Service APP provides clinical consultation in all the domains of Glycemic Control Consult Service.o Patient/family/provider communication about disease and treatment and how it relates to patient values, priorities, and goals of care across the spectrum of care.o Articulates recommendations to colleagues with appropriate and specific language in accordance with current standards of practice, develops individualized goals in accordance with shared outcomes by the primary service.3. The Glycemic Control Consult Service APP collaborates with the primary team.o Makes individualized Glycemic Control Consult Service recommendationso Contributes to the plan of care.o Prescribes medications in accordance with the primary service.o Orders diagnostics/treatments as needed after collaboration with the primary team.o Promotes a collaborative approach to decision making.o Participates in discharge planning meetings to identify and propose solutions for barriers to discharge.

  The base salary range for this position will be: $9,167 - $15,417 per month ($110,004 - $185,004 annually) commensurate with experience and qualifications, or as mandated by a U.S. Department of Labor prevailing wage determination.University of Washington faculty engage in teaching, research and service. This position is a part-time, 12-month appointment (July 1 - June 30 service period) renewed annually with an anticipated start date of March 1, 2024 or after. Faculty with 12-month service periods are paid for 11 months of service over a 12-month period (July-June), meaning the equivalent of one month is available for paid time off.
