General
The Targeted Case Manager’s role is to assist clients in better understanding how to use resources in the community, become involved with their community, and utilize skills to better manage stressful life events in appropriate and healthy ways without the use of substances. The TCM must be self-motivated and maintain excellent documentation regarding each person served. This position will maintain a caseload of no more than 25 individuals and ensure compliance with all HIPPA laws.
Qualifications and Education Requirements
Bachelor’s degree in social work or human services field
1 year post-degree work experience or Master's Degree in a human services field
Must have a current driver’s license

Have a strong working knowledge of computer skills and use of search engines
Be able to handle crisis situations in a calm and supportive manner
Maintain a positive, professional attitude toward clients and staff

Maintain confidentiality and comply with HIPPA laws
Adhere to program policies and procedures
Excellent time management and organizational skills
Provide holistic and comprehensive case management
Maintain a professional code of ethics
Maintain open communication with clients
Document all sessions with clients
Maintain a community contact list
Attend weekly staff meetings or conduct meeting as necessary
Participate in staff training and state regulatory training
Client Related
Create a safe and healthy environment for clients
Have a sincere commitment to client’s recovery
Transport clients to appointments when necessary
Monitor client's emotional well-being and behavior
Connect client with family and community resources
Connect client with legal entities when necessary
Motivate clients toward goal achievement
Meet with clients weekly
Help residents with long-term care plan development and educational resources
