Provides this service in a home setting whileperforming the following duties: Responsibilities Reviews Physician'sreferral, patient's condition and medical records to determine speechtherapy treatment required. Obtains history and subjective components ofthe evaluation In the home setting: Diagnoses and evaluates speech,language, communicative, cognitive, and swallowing skills as related toeducational, medical, social, and psychological factors Plans, directs,or conducts rehabilitative treatment programs to restore communicativeefficiency of individuals with communication problems of organic andnonorganic etiology Provides counseling and guidance and languagedevelopment therapy to patients Reviews individual file to obtainbackground information prior to evaluation to determine appropriatetests and to ensure that adequate information is available Administers,scores, and interprets specialized hearing, speech and language testsDevelops and implements individualized plans for assigned clients tomeet individual needs, interests, and abilities Reviews treatment plan,and assesses individual performance to modify, change, or write newprograms Evaluates swallowing abilities via bedside swallow/videostudies, developing a plan of care based on findings and individualizedpatient needs Maintains records as required by law, establishment'spolicy, and administrative regulations Performs complete, thorough, andtimely electronic documentation Instructs individuals to monitor theirown speech and provides ways to practice new skills Acts as consultantto educational, medical, and other professional groups Confers withPhysician and other practitioners to obtain additional patientinformation, suggest revisions in treatment program, and integratespeech therapy treatment with other aspects of patient's health careParticipates in the PI and CQI process through committees as neededAssist in the orientation and training of new staff Education andExperience A qualified speech-language pathologist holds a Certificateof Clinical Competence (CCC) from the American Speech-Language-HearingAssociation Master's Degree from an accredited Graduate Program.Completion of a Clinical Fellowship under the supervision of a certifiedSLP Home health experience preferred Certification/Licensure LicensedSpeech Therapy in the State of Wisconsin and/or Illinois or registryeligible Approved CPR or BLS certification required within 90 days ofhire Maintain a valid driver's license and...
