  Summary he New Mexico VA Health Care System is seeking a Social Work Program Coordinator responsible for the Veteran Directed Care (VDC) Program. The incumbent will manage the daily operation of the program and provides direct patient care services in the program area to include daily operation of the programs, developing policies and procedures for program operation and preparing reports and statistics for facility, VISN, and national use.. Responsibilities Duties include but not limited to: Provide assessment, treatment, and planning for a complex population of patients which reflect the age-specific, psychosocial, and functional needs of the patient (e.g., spinal cord, geriatric, and mental health population). Participate actively in Veteran's treatment planning processes as a member of interdisciplinary teams as needed, and/or serve on VA committees or workgroups assigned by the GEC Deputy Chief. Develop and implement information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Perform program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, State, etc.). Conduct comprehensive biopsychosocial and suicide assessments. Document social work interventions, mental health diagnoses, and other program related activities in the Veteran's clinical record utilizing CPRS. Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP) - See "Additional Information" below for details. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Licensure: Must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Current state requirements may be found by going to VA's Title 38 Hybrid Occupations - Office of Human Resources Management. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
