JOB REQUIREMENTS: Duties Administers information and analytical systemsto evaluate and enhance the quality of service provided to patients.Performs a wide range of services and provides technical assistance tothe medical center, State, local, and private agencies, anduniversities. Performs work related to the review, monitoring, analysis,planning and assessment of the HUD/VASH program. Independently performscase finding functions and outreach, identifying and screening patientsfor psychosocial needs. Performs clinical social work functions andspecialized clinical social work functions as approved on theincumbent\'s clinical social work scope of practice application.Independently conducts psychosocial assessments. Develops treatmentplans in collaboration with the veteran/family and with theinterdisciplinary treatment team. Coordinates community-based services,including information and referral for additional services from other VAprograms, other government programs and community agency programs.Independently identifies high-risk patients and provides clinical careand services. Establishes and maintains effective therapeuticrelationships with veterans and their families. Independently providesconsultation services to other staff regarding the psychosocial needsand substance use disorder treatment needs of veterans and the impact ofpsychosocial problems and substance use on health care and adherence totreatment plan. General knowledge of veterans\' benefits and servicesrelating to special programs, service connected compensation,non-service connected pension. Performs other related duties asassigned. Work Schedule: Monday-Friday 8:00 am - 4:30 pm Telework: VAsupports the use of telework as a way to help attract and retaintalented individuals in public service, increase worker productivity,and better prepare the agency to operate during emergencies.
