　　The role you’ll contribute:

　　The Social Worker assists in meeting the psychosocial needs of patients and their families, as well as, coordinates concrete services to maximize a patient/family’s quality of life and enhances adjustments to illness. In collaboration with the multidisciplinary oncology team members, the social worker identifies high-risk patients through screening mechanisms, performs assessment of patient/families to determine and facilitate the most appropriate intervention. Maintains a thorough knowledge of hospital, local, state and federal resources. Provides counseling services, including adjustment, supportive and crisis intervention.

　　· Identifies patients/families that require social service intervention via high-risk screening, chart rounds and/or referrals.

　　· Interacts with all newly diagnosed inpatients to assess their needs during hospitalization and post discharge.

　　· Interacts with and provides support to all newly diagnosed outpatients to assess their needs and connect them with resources, if needed.

　　· Identifies the need for and arranges patient/family/health team conferences.

　　· Provides support to the oncology patient/family throughout the continuum of care, critical incident intervention, crisis intervention, and illness adjustment counseling.

　　· Anticipates counseling for individuals, couples, families and groups and facilitates support through the use of resources available to the multidisciplinary team.

　　· Provides social work services based on age-specific needs of the patient and family.

　　· Provides referrals and linkage to concrete services; financial assistance, housing, meals, transportation, and cancer specific mental health services.

　　· Plans and conducts Oncology related support groups and educational programs.

　　· Maintains professional standards adhering to NASW and AOSW Code of Ethics and professional development.

　　· Advocates for oncology patient/family unmet service needs.

　　· Complete projects and reports as assigned by the Director (i.e. patient satisfaction surveys, monthly stats, etc.)

　　· Takes lead role and participates in Oncology related community events.

　　· Provides stress management for the Oncology staff.

　　· Provides Oncology social service expertise in collaboration with other Oncology professionals.

　　· Performs other hospital-related duties as assigned.

　　Qualifications

　　What You Will Need:

　　· Bachelor’s Degree in Social Work or related field (ie., Sociology, Psychology).

　　· Clinically Licensed Social Worker or Masters in Social Work. (Preferred)
