This position will service our regional emergency departments at Toledo Hospital, Flower Hospital, Bay Park Hospital and our Freestanding ED.

　　The Social worker II Basic Purpose: Enhance the quality and effectiveness of the urgent health care services in the emergency center by addressing the psychosocial needs of patients and their families in crisis and providing supportive services to those persons traumatized, victimized unsafe, and coping poorly, or displaced.

　　Other duties include, but are not limited to:

　　Interviews patients, relatives and other to assess their personal and social adjustment an determine necessary social intervention

　　Prepares psychosocial assessment histories at the request of medical staff

　　Counsels patient and their families to enhance their understanding and acceptance of personal and situational problems; helps them to change maladaptive patterns of behavior and to make decisions about problems resulting from illness or disability

　　Assists patient and families with their plans upon hospital discharge

　　Acts as a liaison among patient, family doctor, and nursing staff arranging transfer of patients

　　Refers patients and relatives to community agencies for counseling regarding interpersonal conflict, emotional problems, home health care or rehabilitation and nursing facility placements

　　Master’s Degree in Social Work is required.

　　Must have the following State of Ohio license - LSW or LISW.

　　Must have a minimum of three years’ experience in individual, group and family counseling.

　　Experience in an emergency health care setting preferred. The ability to work effectively with professional in diverse disciplines essential.

　　Must be able to move about hospital and between workstations; and prolonged periods of standing.

　　Must be able to understand directions, communicate and respond to inquiries; requires effective interpersonal skills.

　　Must be able to operate machinery.

　　Must be able to frequently move, lift or carry materials weights up to 30 lbs., 20% of the time. **

　　Must be able to input and retrieve from the computer
