Job Summary
Under general professional supervision, performs social work/casemanagement services including planning, organizing, treatment andinpatient discharge/disposition. Delivers social services in accordance withthe approved plan of treatment and plan of care. Assists the physician andother healthcare members in understanding significant social and emotionalfactors related to the patient's health problems.
Essential Functions:
Provides services which involves identifying client needs, planning forimplementation or coordinating transitional care plans through resourceacquisition, monitoring, advocacy and evaluation in collaboration with theRN Care Coordinators. Conducts psychosocial assessments that areindividualized, objective and complete. Provides services appropriate forpresenting problem. Consults, negotiates, and coordinates with internaland external resources to maximize patient outcomes. (20%)Applies basic knowledge of individual, group and family dynamics andtreatment methods to assist client in achieving treatment goals. Demonstratesability to develop effective interventions that are sensitive tosocio-cultural factors. (20%)Participates in multidisciplinary patient care meetings (i.e. careconference & rounds) and utilizes information to assess and reassess careneeds. Develops working knowledge of programs and eligibility of the programs.Demonstrates skill in obtaining appropriate health care resource/servicesand educating others. Participates in staff meetings, staff development andapplies knowledge of groups and their tasks through appropriate participation.Monitors and evaluates effectiveness of services to consumers and modifiesintervention plan as indicated. Professional activity complies with applicablelaws. (15%)Assists clients to maximize adjustment to illness/disability/trauma byaddressing biopsychosocial issues and enhancing problem solving and copingskills. Evaluates emotional components, impact of health issues and assistsclient to identify disruptive factors to establish treatment goals. (5%)Adheres to and complies with rules and regulations of the City, Agency,Department and Service. Attends all mandatory meetings and applies knowledgein daily practice i.e. safety and infection control and identified areasaffecting practice. Practices in accordance with ethical principlesestablished in NASW code of ethics and by Denver Health. (5%)Completes documentation of social work activities and interventions perestablished standards and guidelines. Applies written guidelines to assurework is accurate and complete. Prepares and submits data and information toCSW in a timely manner. Chart notes are legible, thorough, and timely andcontent reflects established qualitative standards. Documented activityreflects patient involvement in goal setting with written informed consentdocumented. (5%)Utilizes methods, techniques and approaches to the delivery of care that areconsistent with the patient population and in conformance with the specificcare plan developed for each patient. (5%)Develops knowledge base of risk factors for psychosocial functioning,wellness and health promotion and provides basic education to patient. (5%)Education:
Master's Degree RequiredWork Experience:

Knowledge of emotional and social factors that effect patient's health.Knowledge of discharge and in-service education programs.Excellent interpe sonal skills.Skilled in identifying problems/opportunities to improve care.Ability to help the patient and family to understand, accept, and followmedical recommendations.Ability to utilize resources, such as family or community agencies, toassist the patient resume life in the community.Ability to collect key information from the patient, family, and thecommunity, as part of the social work assessment.Ability to communicate to the appropriate staff, information about thepatient's discharge plan.
