Assist the Director of Admissions and Social Work as an advocate for residents and families. Complete documentation as required by both the facility and New York State guidelines and regulations. Essential Duties & Responsibilities: *Work within an Interdisciplinary Care Team environment to provide for the psychosocial needs of both short-term rehabilitation and long-term care residents. *Communicate effectively and develop working relationships with the families and designated representatives of residents, in areas such as transition to placement, dementia education, internal and external support resources. *Attain a working knowledge of Person-Centered Care Planning and documentation practices to meet NYSDOH and CMS regulatory requirements. *Attend Interdisciplinary Team Meetings and Resident Care Conferences, as required. *Assist the Director in Quality Assurance and Performance Improvement (QAPI) initiatives. *Participate in admission and discharge planning duties, as required. *Attend Resident Council Meetings and Family Council Meetings. *Assist Director of Social Work with addressing concerns, and also with Council projects. *Assist the Director in responding to any concerns, grievances, reported lost/missing items; assist as liaison to the Long-Term Care Ombudsman program. Provide support and referral to residents, families and designated representatives regarding financial and administrative needs, as well as insurance navigation. *Work effectively within a campus environment, developing a working knowledge of services offered throughout the organization. *Assist as needed in infection-control practices and education throughout the facility. *Performs other duties as assigned that are necessary to meet the needs of the residents and their families. Education Required: Bachelors degree in Human Services or related field, with 2 years of experience in a long-term care setting preferred OR Associates Degree in Human Services or related field required, with minimum 5 years of experience in a long-term care or human services setting. Ability to read and write English with demonstrated oral and written skills to meet professional and regulatory requirements is required. Ability to use multiple computer programming platforms; previous experience with an Electronic Medical Record (EMR) preferred.
