　　Are you a Licensed Social Worker looking to make a difference? We need you!

　　The LSW will help senior long term care and transitional care unit residents and families to understand and cope with the psychosocial aspects of their lives involving change, grief, and loss through crisis intervention—individual, group and family counseling. We are looking for an individual to coordinate discharge to an alternate level of care as appropriate. Bachlors in Social Work position will be serving Jones-Harrsion skilled nursing care center in Minneapolis, MN near beautiful Cedar Lake.

　　This full time Social Worker role includes:

　　Full Time Hours; Days, 80 Hours Per Two Week Pay Period
