About Service Coordinator

　　Primary Functions: Provide case management services including needs assessment, service plan and monthly contacts.Develop and coordinate community wide outreach for youth.Develop plans of intervention that address needs for housing, services, crisis and intervention.Link clients to available resources to assist with meeting basic needs and progress, coordinate and facilitate with treatment team to assist client with reaching person centered goals.Network with agencies, coalitions, and local community meetings.Shift/Days and Work Hours: Monday-Friday (flexible schedule with some work at home time)
