Seeking a Registered Nurse, to complete start of care Admissions for our Home Health department in the St. Louis, MO and surrounding areas!

　　As a Phoenix Admissions RN, you will be the first point of contact between a patient and hospital. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

　　Availability: Full Time, Monday - Friday

　　Pay Range: $75,000 - $90,000

　　Responsibilities:

　　Skilled Visits

　　Day-to-day patient interaction

　　Education on Health Conditions, such as CHF, CODP, Diabetes

　　Wound Care

　　Medication Management

　　Changes dressings, bandages and contraptions as prescribed

　　Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.

　　Working alongside other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist.

　　Requirements:

　　Missouri or Compact Nursing License

　　Strong organizational and self-management skills

　　Valid driver’s license
