  Dept: 3E Intermediate Care Unit

  Location: SMH

  General job duties. Types of procedures required.3E IMC is a 38 bed unit that specializes in stepdown care of medical, surgical, neurology, or trauma patient. Patients on the unit require a level of care that is higher than a general medical unit but not intensive care level. IMC patients could require continuous cardiac monitoring, titratable medications, mechanical ventilation and vasoactive infusions. All beds have on-site telemetry monitoring, which is monitored by tele technicians. Most patients range from young to older adults with at times adolescent and geriatric patients. We are a fast paced unit with many higher level acuity needs with both short and long stays.

  Nursing Skills developed on the unit include - Central lines, TPN, pacemaker care, drain care, chest tubes, tracheostomy, IVs, EVD, PCAs, dressing changes, NGs, medication management and post-surgical care.

  Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)Electronic Health Record - Cerner System and use of bedside bar code administration

  Defibrillators, telemetry, bed scales, stand up scales, infusion pumps, arterial lines, IV equipment, suction equipment, central lines, capnography, ventilators, and CPAP/bipap.

  Required skills and minimum years of experience.Prefer 1 year of med-surg nursing; cardiac experience is helpful

  Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)Current IL license required as well as BLS, ACLS preferred

  Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)Shifts are 0645-1915 and 1845-0715

  Every other weekend rotation is expected.

  no on call requirements

  Employee will always work with a team or nurses, patient care technicians, and a unit secretary

  Floating to other units/departments. (If so, to which units/departments?)Will need to be flexible to float first, then based on hospital needs would float to other units based on hospital needs and individual competency.

  Bed size/number of rooms for this unit/dept.38 all private rooms, all are IMC level of care.

  Nurse to patient ratio of this unit. (nursing only)1:4

  Dress Code:any color scrubs

  Unit 3E Intermediate Care

  # Beds: 38; all private; all tele-monitored beds

  Ratios: 1:3-4. Usually 4. Never 5.

  Common diagnosis / Types of patients Sepsis, COPD/CHF exacerbation, Client, stroke, post-Covid, respiratory distress, trauma (falls, GSW, MVA), pancreatitis

  Experience or 'Must have' skills Trach/vent (vent is managed by RT and can be taught on unit), titratable drips (no sedation or pressors), neuro (stroke/TBI/brain bleed/seizure), Optiflow/cont bipap, wound management

  Are RN's required to titrate drips Yes. No sedation or pressors.

  Common titratable &/or set rate drips Diltiazem, amiodarone, heparin, nicardipine, aztreonam, Lasix, nitro

  Tele/Monitor Techs on this unit? Do RN's read strips? Tele techs 24/7. RNs do not need to print/analyze strips. Must be able to interpret for documentation.

  Charge Nurse/House Supervisor Try to maintain a free charge nurse. NM Mon-Fri, HS after hours and on weekend.

  Are there Nurse Aides on this unit/Ratios 3-4 techs/shift. 8-12 patients.

  RT/Equipment (ie Cpap, Bipap, High Flow) Cpap, Bipap, Vent, OptiFlow

  Shifts & Scheduling: Every other weekend. Flexibility with scheduling is very important. Blocks not guaranteed.

  Weekend Requirement EOW

  Holiday Requirement May be scheduled as needed.

  Call or Standby Requirement No.

  Floating Requirements Float in rotation (rare)

  Scrub Color/Dress Code No scrub color. Clean and professional.

  Unit/Department Specific Orientation (2) 12 hour shifts on unit after 2-day onboarding

  Additional Notes Level I trauma center, teaching hospital, quadruple Magnet.

  Busy, fast-paced unit. Part of critical care team. Higher acuity than a lot of stepdown/PCU units. Bedside shift report. Excellent unit rapport with travelers. Most travelers extend several times. Unit is <7 years old with large private rooms. Easy to work in from nursing standpoint. Scheduled meds and daily supplies delivered to each patient room.

  *Prefer previous travel experience as this unit is extremely busy. Telemetry unit experience is generally not adequate. Prefer step-down/IMC or ICU experience.
