　　Job Description/Essential Functions:

Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.We are seeking a Full-Time Rehabilitation Counselor to provide support, education, counseling and guidance to child(ren) or adolescent(s) who has been identified as having mental health or developmental disabilities. Responsibilities for this position include:Assist participants with goal achievement and local community interaction.Implementing the overall mission, values and beliefs of Horizon House in all aspects of the job.Assisting with the growth and development of the participant and the development of therapeutic support skills.Providing one-on-one behavioral intervention in the home, school, or community settings in accordance with strategies identified in the Individual Treatment Plan.Participating in developing goals with the participant for inclusion in the individualized service plan.Ensuring provision of preventive healthcare and necessary follow-up services for participants.

　　Job Qualifications:

　　Bachelor's Degree in Human Services/Social Services, Healthcare or related field. At least 1 year of experience working directly with children; or H.S. Diploma or GED equivalent plus 12 behavioral health college credit hours from an approved educational program plus a minimum of 2 years experience (paid or unpaid) in a human service field. The experience must involve direct contact with the individual receiving services (i.e. coaching, teaching, case management, etc.).Experience in residential or other services to children with MR/MH preferred.Good communication and counseling skills.Ability to work independently and as part of a focused team.Proficiency in various computer technologies including Electronic Health Record, Outlook, MS Word and Excel.Fee-for-service experience required and ability to maintain daily productivity standards using an Electronic Health Record.Possess a valid driver's license with an acceptable driving record.SCOPE:Frequent talking and listeningRequires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.Possess manual dexterity and fine motor skills.May require crisis intervention to emergency situations.To be considered an applicant for this position please complete the pre- employment application by visiting us at www.hhinc.org/careers.
