　　GENERAL SUMMARY:

　　Provides leadership in the coordination and delivery of quality compassionate patient care. Utilizes the nursing process of assessment, planning, implementation, and evaluation, to provide, delegate, supervise and document care and teaching of patients and family. Effectively communicates pertinent patient/family information to health care team. Participates in finding solutions for variances from patient goals and ensures implementation of solutions. Over the IRS MIleage Reimbursement rate.

　　EDUCATION & EXPERIENCE:

　　Requires graduation from an accredited school of nursing, and current Michigan licensure as a Registered Nurse.

　　Hospice certification preferred.

　　A working knowledge of current pain and symptom management, and experience appropriate to the needs of hospice patients and their families is preferred.

　　Requires interpersonal skills to present a positive image of Hospice to those whom are referred to the program.

　　Requires the ability to be flexible in adapting policies and procedures to meet the needs presented by Hospice patients and families.

　　Requires a valid drivers license, an acceptable driving record, and reliable transportation.
