Responsibilities

• Implement the treatment and rehabilitative strategies outlined in the patient care plan, including medication management, rehabilitation treatments, coordination of medical needs, and budget monitoring• Maintain thorough and timely documentation of care provided to ensure accurate patient record and billing• Conduct assessments and approve plan of care and service delivery methods• Follow consumer health plans, provide and promote consumer choice, and facilitate consumer independence• Perform competency and skills evaluations on employees who provide direct care• Other tasks as assigned

Qualifications

• Must be at least 21 years of age• Active Registered Nurse (RN) license in good standing in state of operation• Bachelor’s Degree in nursing or related health and human services field from an accredited college or university OR an Associates Degree in nursing from an accredited two-year program
