POSITION SUMMARYThe Registered Nurse is a licensed health care professional with advanced medical knowledge whoadministers skilled nursing care to patients requiring intermittent visits. She/he provides complexcare to the patient and coordinates necessary support services. These nursing services are providedto patients in accordance with a physician's plan of care. Registered Nurses who serve as primarynurses utilize a case manager approach in assessing, implementing, coordinating and evaluatingclient care. Additionally, the Registered Nurse manages other members of the nursing team.
QUALIFICATIONS• Registered professional nurse with a current valid Pennsylvania license• Minimum of two [2] years of current experience as a professional nurse in amedical/surgical work area and one [I] year of home health experience (preferred)• Skilled in critical thinking, communication, negotiation and collaboration• Reliable, independent means of transportation to make home visits. Drivers must have avalid driver's license and drive a vehicle with insurance limits as established by the state.DUTIES AND RESPONSIBILITIESClinical SkillsA. Provides skilled nursing care that includes activities such as teaching, treatments,(including IV therapy, venipuncture, complex wound care) and health promotionpracticesB. Executes preventive, rehabilitative and restorative nursing procedures appropriate for thepatient's care and safety
C. Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs,and changes in patient's physical, mental, or emotional condition to the physicianD. Develops the plan of care collaboratively with the physician, patient, designatedrepresentative. and the agency with revisions made based on patient conditionE. Instructs and counsels the patient and the designated representative regarding the carerequirements of the patient in the homeF. Assigns and instructs the Licensed Practical Nurse (LPN) or home health aide inaccordance with the physician's plan of care1. Supervises and evaluates the care given by the home health aide to his/her primary patientsevery 14 daysI-I. Supervises and periodically evaluates the care given by the LPN to his/her primary patients toensure oversightI. Evaluates and manages LPNs and home health aides who are providing services lo his/herprimary patientsJ. Obtains a medical history, particularly as it relates to the present condition, from the patientand/or designated representative at the start of care.K. Conducts a comprehensive physical examination and nursing assessment of the patient aleach skilled visitL. Evaluates the patient and the home situation to determine the health teaching required at each skilledvisitM. Evaluates the patient's condition and home situation to determine what assistance will be availablefrom the designated representative in caring for the patientN. Coordinates total patient care serving as the primary nurse for designated patients andevaluating the effectiveness and the appropriateness of care0. Interprets nursing and other services of the agency to the patient and designatedrepresentative as part of planning for careP. Serves as the primary agency liaison and coordinator of care when nursing and other services arcprovidedQ. Plans with the patient, designa ted rep resenta ti ve, and other team members for thepatient's discharge from serviceR. Meets with the interdisciplinary team routinely to ensure coordination of care and to update theplan of care as appropriateS. Meets with Clinical Supervisor or Case Ma nager weekly for an evaluation of patients under his/her care
T. Completes all appropriate OASIS assessments and documentation as assigned
