Job Description:

　　The Clinical Dietitian, under the direction of the Chief Clinical Dietitian, is responsible for all areas of patient screening, assessment, monitoring of nutrition therapies, and education of patients and/or family members.

　　Must be proficient in providing nutrition care to all types of patients including pediatric, geriatric, oncology, diabetic, renal, cardiac, and acute patients.

　　The dietitian is responsible for education of staff (inter- and intra-departmentally) on nutrition related issues and for establishing and maintaining effective communication intra-departmentally.

　　Performs nutrition assessments and reassessments to assure that patients' nutritional needs are met (includes initial assessment, on-going reassessment, and discharge assessment/planning).

　　Utilizes all avenues of data-gathering (i.e., old charts, patient's family, previous nursing home, etc.) to provide the most comprehensive nutrition care possible.

　　Sets nutrition goals and education goals at initial assessment and monitors patient progress towards goals at reassessment.

　　Also documents patient's response to nutrition interventions at each follow-up.

　　Schedule - Day Shift (variable between weekdays/weekends)
